Birth and Death Certificates

BIRTH AND DEATH CERTIFICATES

Summit County Public Health provides certified birth certificate copies for every person born in the State of Ohio and certified copies of death records for every person who died within Summit County. The fee for all certified certificates is $22.00 per copy. Checks and money orders should be made payable to Summit County Public Health. Credit and debit cards are also accepted (additional fees apply).
