Employee Access

Community Health

The Division of Community Health has four main focus areas: population-based prevention, care coordination and case management, early childhood and access to healthcare.  We employ a multi-disciplinary approach to service provision, utilizing registered nurses, licensed social workers, and health educators.

The Division of Community Health provides a leadership role in health and policy development for the County.  This involves working in partnership with a number of key stakeholders including community organizations, local and state government, public education, hospitals, and consumers.
